Template for Library Information Sheets

Our new publishing system, Academic Archive Online (DiVA) does not generate a Library Information Sheet. With the new system it is not necessary to include a Library Information Sheet in your thesis since the system automatically sends a text file to the University catalogue with all bibliographic information you have filled in by the web form. If your department still would like to use the Library Information Sheet you may use the template below.
   You can fill in the document directly on the screen, save it on your computer with a new name, insert it in your thesis or print it out.

If you have a long abstract there will perhaps be no room for all the information on one page in the Library Information Sheet. In order to make more room, use a smaller font size. Instead of 12 p, lower the size to 10 p or lower. The text must, however, be readable so do not lower it too much. It looks much better if the library information sheet consist of only one page.
